This project consists of a 75,400 square feet, two story 50 - unit apartment building. The facility is located in the Town of Wheatfield, New York. The facility contains 36 singlebedroom apartments and 14 two-bedroom apartments located on property adjacent to the Crestwood Health Care Center and the Crestwood Common Patio Homes.

The buildings general layout includes two resident wings with a centrally located common area. The common area includes a resident dining room with a fully equipped commercial kitchen. The common area also includes lounges, social gathering spaces and a spacious outdoor patio area.
